This referral pathway explains two routes, depending on the level of support needed.

- Requesting a statutory Young Carers Needs Assessment

Information for Young People
If you are under 18 and think you might be a young carer, we would love to support you 💙
Accessing Young Carers Activities and Support
- Ask for help: Speak to a trusted adult such as a parent/carer, teacher, or support worker and ask them to complete a Young Carers Screening Tool with you 📝
- This helps us understand what your life looks like and how you help at home
- Once completed, the screening tool is sent to Slough Children First, where it is logged and passed to the Young Carers Lead at Slough Young Carers (a part of Together As One).
- Welcome aboard: We will contact you to help you get started with our sessions and activities 🎉
⚖️ Your Rights
- Under the Care Act 2014, young carers are entitled to a formal Young Carers Needs Assessment
- If you would like a formal assessment, you can ask the Young Carers Lead or a supporting professional for help
- Taking part in young carers support is voluntary and does not require the involvement of social services unless there are wider concerns

If you believe your child is a young carer, they can access support in the following ways:
Accessing Young Carers Activities and Support
- Complete a Young Carers Screening Tool with your child to help us understand their caring role and its impact 📝
- The screening tool is sent to Slough Children First, where it is logged and passed to the Young Carers Lead 📤
- The Slough Young Carers Team will contact you to explain the activities and support available 📞
📋 Requesting a Young Carers Needs Assessment
- Under the Care Act 2014, young carers have the right to a formal Young Carers Needs Assessment ⚖️
- Parents and carers can contact Slough Children First directly on 01753 875 362 to request an assessment ☎️
- Slough Children First will review the request and complete the assessment, including the screening tool as part of the process 🔍
Information for Professionals
Teachers, Schools, Health & Community Professionals
- Consent: Please obtain consent from the parent/carer before completing a referral ✅
For Access to Young Carers Activities and Support
- Complete the Young Carers Screening Tool 📝
- Send the completed tool to Slough Children First via the Young Carers inbox
- Slough Children First will log the screening and pass it to the Young Carers Lead at Slough Young Carers (a part of Together As One), who will contact the family.
📑 For a Statutory Young Carers Needs Assessment
- Complete the Young Carers Screening Tool and a MARF 📄
- Submit both documents to MASH for review and allocation
Social Workers & Targeted Early Help Practitioners
- Complete the Young Carers Screening Tool 📝
- Ensure it is recorded on the Slough Children First system 💻
- Send the screening tool to the Young Carers inbox so it can be logged and passed to the Young Carers Lead for support
- A MARF is only required where a statutory assessment is being requested or where there are safeguarding concerns

Once the Young Carers Screening Tool has been logged by Slough Children First and passed to the Young Carers Lead, a member of the Slough Young Carers Team (within Together As One) will contact the family to explain the activities and support available
